OUTREACH SERVICES
CORE Association provides supports to adults with developmental disabilities through Outreach. Supports are based on a support plan that identifies where individuals may wish to develop or maintain skills as well accessing their community and building community connections.
In Home Respite
Individuals are supported in their familial home or in a support home, usually evenings or weekends.
Out of Home Respite
Individuals can spend a minimum of 24 hours in a family-type atmosphere. Normal routines are expected to be followed, with an emphasis on participation in activities in the community.
Supported Independent Living
Individuals are supported to live in their own homes or with peer room-mates. Skill development focuses on independent living skills, assistance with navigating community resources and building community connections.
Support Homes
An individual may live with a family or supportive room-mate.
